Note: Visa Application Centers in Kyiv, Kharkiv, Dnipropetrovsk, Lutsk, Rivne, Ternopil, Lviv, Ivano-Frankivsk and Uzhgorod will be working on Saturday, 23 May 2015 from 08:30 till 15:30 for the application submission from the applicants applying for visa for the football match only and passport collection. Visa Application Centres in Odessa, Vinnytsya, Khmelnytskyi, Zhytomyr and Chernivtsi will be closed on this date

The Embassy of Poland is pleased to announce the Visa Application Centre operated by VFS in Lviv, Kiev, Odessa, Ternopil, Ivano-Frankivsk, Rivne, Khmelnytskyi, Zhytomyr, Dnipropetrovsk, Lutsk, Kharkiv, Vinnytsya, Chernivtsi and Uzhgorod. This service is available for Ukrainian residents/other country nationals legally residing in Ukraine who wish to apply for a Schengen visa to Poland. This site provides with details related to your application submission and collection process along with other useful information you need to know.

All applications collected by the Visa Application Centers will be sent to the Consulates General of Poland in Ukraine for assessment and resolution. Visa Application Centre staff has no say in the visa decisions taken by the Consul.

You may schedule an appointment 24 hours post payment conducted at the respective bank. Unique code is activated during working days.
The unique number cannot be used for scheduling an appointment twice to Poland Visa Application Centers in Lviv, Ivano-Frankivsk, Uzhgorod, Kharkiv and Dnipropetrovsk, Vinnytsya, Zhytomyr, Chernivtsi, Khmelnytskyi and Kyiv. In case registration was deleted unique numbers is deactivated and cannot be used to re-register.

Track Your Application
After submission, you can also check the status of your application online by simply providing your unique VAC Reference Number and date of birth (using the format dd/mm/yyyy). VAC reference number (starts with POIV, POLV, POTE, PORI, POLU, PODN, POKH, POKI, POOD, POKY, POZH, POVI, POUZ, POCH) can be found on the Invoice-Cum-Receipt provided by the Submission Officer at the Visa Application Centre at the time of submission.
