The Embassy of Poland is pleased to announce the Visa Application Centre operated by VFS in Lviv, Kiev, Odessa, Ternopil, Ivano-Frankivsk, Rivne, Khmelnytskyi, Zhytomyr, Dnipropetrovsk, Lutsk, Kharkiv, Vinnytsya, Chernivtsi and Uzhgorod. This service is available for Ukrainian residents/other country nationals legally residing in Ukraine who wish to apply for a Schengen visa to Poland. This site provides with details related to your application submission and collection process along with other useful information you need to know.

All applications collected by the Visa Application Centers will be sent to the Consulates General of Poland in Ukraine for assessment and resolution. Visa Application Centre staff has no say in the visa decisions taken by the Consul.

To visit Visa Application Centre you need to Schedule Appointment. You can book an appointment online or by calling the Call Centre.
Important to note: in case if the available appointment date online is not suitable for you, you also schedule an appointment via Call Centre. To check closest available dates at the Call Centre – please click here.
Please be informed that in case of registration through Call Centre you will be also able to get information concerning documents and visa applying process.

Visa Application Process
Consulate General in Lviv is pleased to announce that from 1 June 2014 the process of registration to Poland Visa Application Centers in Lviv Ivano-Frankivsk and Uzhgorod will change.
Consulate General in Kharkiv is pleased to announce that effective 17 November 2014 process of registration to the Poland Visa Application Centers in Kharkiv and Dnipropetrovsk will change.
Consulate General in Vinnytsya is pleased to announce that effective 24 November 2014 process of registration to the Poland Visa Application Centers in Vinnytsya, Zhytomyr, Chernivtsi and Khmelnytskyi will change.

NOTE: The unique number cannot be used for scheduling an appointment twice to Poland Visa Application Centers in Lviv, Ivano-Frankivsk, Uzhgorod, Kharkiv and Dnipropetrovsk, Vinnytsya, Zhytomyr, Chernivtsi and Khmelnytskyi. Service fee will not be refunded after your appointment is cancelled.

Track Your Application
After submission, you can also check the status of your application online by simply providing your unique VAC Reference Number and date of birth (using the format dd/mm/yyyy). VAC reference number (starts with POIV, POLV, POTE, PORI, POLU, PODN,  POKH, POKI, POOD, POKY, POZH, POVI, UZHG, POCH) can be found on the Invoice-Cum-Receipt provided by the Submission Officer at the Visa Application Centre at the time of submission.
