Introduction
Please be informed that during prepayment and appointment scheduling process all the data provided by you should be verified and be absolutely correct in particular:

Year and date of birth, passport number, name and surname, visa category and visa centre.

In case during application submission at the VAC data will be verified and it will be stated that provided during initial processes information does not match with actual information such applicants will not be accepted by the Visa Centre.

Note that it is required to come to the VAC for document submission at the exact time chosen while appointment schedule. Applicants who came to the VAC on time will be accepted on priority.

The Embassy of Poland is pleased to announce the Visa Application Centre operated by VFS in Lviv, Kiev, Odessa, Ternopil, Ivano-Frankivsk, Rivne, Khmelnytskyi, Zhytomyr, Dnipropetrovsk, Lutsk, Kharkiv, Vinnytsya, Chernivtsi and Uzhgorod. This service is available for Ukrainian residents/other country nationals legally residing in Ukraine who wish to apply for a Schengen visa to Poland. This site provides with details related to your application submission and collection process along with other useful information you need to know.

All applications collected by the Visa Application Centers will be sent to the Consulates General of Poland in Ukraine for assessment and resolution. Visa Application Centre staff has no say in the visa decisions taken by the Consul.

Note:
You may schedule an appointment 24 hours post payment conducted at the respective bank. Unique code is activated during working days.
The unique number cannot be used for scheduling an appointment twice to Poland Visa Application Centers in Lviv, Ivano-Frankivsk, Uzhgorod, Kharkiv and Dnipropetrovsk, Vinnytsya, Zhytomyr, Chernivtsi, Khmelnytskyi, Kyiv and Odessa. In case registration was deleted unique numbers is deactivated and cannot be used to re-register.

Track Your Application
After submission, you can also check the status of your application online by simply providing your unique VAC Reference Number and date of birth (using the format dd/mm/yyyy). VAC reference number (starts with POIV, POLV, POTE, PORI, POLU, PODN, POKH, POKI, POOD, POKY, POZH, POVI, POUZ, UZGH) can be found on the Invoice-Cum-Receipt provided by the Submission Officer at the Visa Application Centre at the time of submission.
